Unable to import users into CRM with CSV file

I attempted to import a csv file into CRM, there are no error messages showing after importing, yet none of the data is imported into CRM.

I downloaded the sample csv file and inserted these information. Does anyone know how to fix this problem? Below is the data inside my csv file.
